During these appointments, we have an opportunity to review with the parents issues regarding general health, immunizations, nutrition, behavioral problems, school and learning problems and other psycho-social or developmental issues that arise. We feel strongly that complete physicals done by the primary care provider with a parent present are in your child’s best interest.
Note: Additional Concerns During Well Visit Effective October 1, 2015: Changes in legislation require that any care provided outside of a standard well check up will need to be documented and billed separately and will involve a copayment and or deductible payment.
We use CHADIS so make sure complete your child’s SURVEY thru patient portal before arriving at your appointment.
Well visits can be made up to 12 months in advance conveniently thru our Patient Portal or calling our office.
How To Request a Check-Up Appointment Through the Portal Video Tutorial
CHADIS is an online system that delivers questionnaires that help your child’s doctor review the health and development of your child. Your child's doctor uses the questionnaires to help determine if your child is developing on schedule and without health risks. These questions can help detect issues early, before some signs are present. When issues are caught early, children are able to receive help earlier and treatment is often more effective. After you have confirmed your appointment you may log into your patient portal and complete the questionnaire specific to your child’s visit. ABC Pediatric Clinic — CHADIS Survey
